Author Forms & Declarations
Official documents used to record authorship, originality, ethical compliance, permissions, disclosures, consent, and publication rights for manuscripts submitted to the Journal of Digital Health and Advanced Biomaterials — JDHAB.
The corresponding author must ensure that all required documents are complete, accurate, current, and approved by the relevant authors, participants, institutions, or rights holders.
Complete one set of required documents for each manuscript submitted to JDHAB. The Author Declaration and License to Publish and the Cover Letter are required for every submission; consent and permission documents are required only when applicable.

Author Declaration and License to Publish
This form confirms originality, authorship approval, ethical compliance, permissions, competing interests, integrity of the submitted materials, and the license terms that apply if the manuscript is accepted for publication.
All authors must review and approve the information that concerns them. The corresponding author must ensure that the completed document accurately represents the entire author group.

Cover Letter to the Editor
This letter introduces the manuscript to the Editor and explains its relevance, original contribution, suitability for JDHAB, ethical compliance, related or overlapping work, and approval by all authors.
The letter should identify the article type and disclose any information that may be important for editorial assessment but does not belong in the blinded review file.

Required When Applicable
Consent for Publication of Identifiable Material
Required when a manuscript contains photographs, imaging, audio, video, clinical information, or any combination of details through which a patient or research participant may be identified directly or indirectly.
Consent for participation in research or treatment does not automatically constitute consent for publication. Specific written consent for publication must be obtained from the individual or legally authorized representative when identification is possible.

Third-Party Material Permission Form
Required when a manuscript reproduces or adapts copyrighted figures, tables, photographs, diagrams, questionnaires, scales, screenshots, dataset excerpts, or other material owned by a third party, unless the intended use is already authorized by a compatible license or applicable law.
The permission must cover the form of use proposed in the manuscript, including publication under the journal’s open-access license when required.

How to Complete the Documents

Download the Current Form
Use the official file available on this page for the manuscript being submitted. Do not reuse a completed form from another submission.

Complete All Applicable Fields
Provide the manuscript title, article type, author information, declarations, approvals, permissions, and other details requested in the document. Do not delete required statements.

Verify Author Approval
Confirm authorship inclusion and order, the submitted version, contributions, competing interests, funding, ethical information, and all other declarations with the complete author group.

Upload the Submission Documents
Upload the completed Author Declaration and Cover Letter with the submission. Include applicable permission and supporting documents without unnecessary patient- or participant-identifying information and in accordance with the editorial workflow.
Declarations in the Manuscript
Forms Do Not Replace Manuscript Statements
Completing the author forms does not remove the requirement to include all applicable declarations in the identified manuscript. These may include ethics approval, consent to participate, consent for publication, competing interests, funding, author contributions, data and code availability, artificial intelligence use, registration, and related-work disclosures.
Consistency Across Files
Information in the forms, identified manuscript, blinded manuscript, submission metadata, reporting checklists, and supporting documents must be consistent. Any discrepancy should be corrected or explained before submission.
Privacy, Retention, and Editorial Verification
Identifiable Consent Records
Original patient- or participant-identifying consent documents must not be uploaded through the routine submission workflow unless JDHAB specifically requests them through an appropriate secure process.
Author Record Retention
Authors must securely retain original consent forms, permissions, licenses, institutional approvals, and supporting records for the period required by their institution, applicable law, funder, or research policy.
Confidential Verification
JDHAB may request confidential verification of consent, permission, authorship approval, ethical review, registration, or other declarations when reasonably necessary. Authors should provide only the information required for that verification.
